ABSTRACT
The context within which hospital planners must work is changing rapidly. A large number of factors are at work which bear on the search for the ‘right’ balance of activity between hospitals and other providers. Some of these tend to support present patterns of provision by improving the services which the acute hospital has to offer, others to undermine it by making it easier to effect substitution: some forces make for greater concentration, others for dispersal.
