ABSTRACT
The Fishbone Diagram, also known as a ‘Cause and Effect’ diagram shown in Figure 1.8, was developed by Dr Kaoru Ishikawa (1968). It is a simple problem-solving tool often used by quality assurance and continuous improvement teams. The model is used to explore, identify and display the potential root causes of a specific effect – usually a quality outcome or a specific defect of some kind.
