The dilemma of prescribing drugs during lactation lies in the decision in terms of which is more important – the potential risk of the drug in breastmilk or the lack of breastmilk? Before prescribing/recommending a drug we need to consider some questions:

How essential is the need for this particular drug if a mother is breastfeeding?75

Is it possible to substitute it with a drug for which we have more data in terms of its passage from mother to infant?

Do we know whether it would be safe for her infant if the mother continues to breastfeed while on this drug?

Would she choose to continue breastfeeding without any medication? Is this decision sensible taking into account her medical condition?
