Although clinical Connected Communities of Care (CCC) workflows will vary across your selected clinical sites, they need to converge on the common goals of the CCC. Numerous nuances and key factors to consider in establishing the clinical consortium of your CCC include executive sponsorship, clear definition of roles and responsibilities, handling of clinical information, compliance framework, and integration of the new workflows. The purpose of the Clinical Providers Track is to set out the stakeholders and processes required to integrate clinical entities, insights, programs, interventions, strategies, and measurement for the CCC.
