ABSTRACT
This chapter provides both straightforward and more complex case scenarios, which relates to some of the Long-Term Conditions (LTCs) in primary care. Part one considers care management issues for people with a specific LTC. It also explains the respiratory conditions, and musculoskeletal conditions and skin condition. The respiratory conditions include asthma, chronic obstructive pulmonary disease (COPD) and diabetes. The suggested answers to the asthma scenarios are based on information taken from the latest edition of the British Thoracic Society (BTS)/Scottish Intercollegiate Guidelines Network (SIGN) Guideline on the Management of asthma. The musculoskeletal conditions include osteoporosis and osteoarthritis. The skin condition includes eczema. Part two considers care management issues for those with more complex needs. The scenarios in part two include Spinal Cord Injury (SCI), and priorities of care and psychological support and counselling. The chapter then discusses the pause for reflection and suggested course of action for each scenario.
