ABSTRACT
Some of the case histories presented in previous chapters revealed that confined space entrants can be killed or injured when hazardous liquids and gases leak into a space through attached pipes. Other cases discussed fatalities that resulted when mechanical equipment such as blenders, mixers and stirrers started unexpectedly.
