ABSTRACT
Conventional middle-class populations receive the best care our present conventional medical institutions can supply. Middle-class people pay their bills on time or have health insurance, which does it for them. They trust the doctor to do what is best for them. They have diseases that go with respectability, and the doctor who treats them need not feel tainted by associating with people whose medical problems arise from activities that are illegal or immoral.
