How was health reform constructed ideologically during empire present?1 During earlier historical periods, discussions of health reform arose periodically. Most of the concepts that received attention later-effi ciency, effectiveness, costbenefi t analysis, freedom of choice, decentralization, community participation, etc.—also appeared during the 1950s and 1960s, to motivate transformations in health-care organization.2
