ABSTRACT
On Lok Senior Health Services of San Francisco, a nationally recognized innovator in long-term care, enables frail elderly persons who are certified for nursing home care to remain in their community for as long as possible. On Lok combines primary medical, adult day health, and homecare services—complete with transportation. Although the designated client (called “participant” at On Lok) is the elderly individual, in reality On Lok provides not only eldercare but family care, because care at On Lok is usually given “until death do us part.” Most participants leave On Lok only at death, after four or more years in the program.
